Please read this document carefully before receiving any services at NUI Nails & Spa. By signing this waiver, you acknowledge that you have read, understood, and voluntarily agree to the terms below.

I certify that the information provided in this form is true, complete, and accurate.

I understand that nail, manicure, pedicure, spa, and beauty services are cosmetic in nature and are not medical treatments. I acknowledge that it is my responsibility to disclose any allergies, medical conditions, medications, or other health concerns that could affect my service.

I agree to immediately notify my technician if I experience pain, discomfort, burning, irritation, or any unusual reaction during my appointment.

Assumption of Risk

I understand that, despite NUI Nails & Spa following professional sanitation, hygiene, and safety standards, cosmetic services involve certain inherent risks, including but not limited to:

  • Allergic reactions
  • Skin irritation or redness
  • Product sensitivity
  • Cuts or abrasions
  • Infection
  • Damage to natural nails
  • Lifting, cracking, or breakage of enhancements
  • Unexpected reactions related to undisclosed medical conditions or allergies

I voluntarily assume all risks associated with receiving these services.

Release of Liability

To the fullest extent permitted by applicable law, I voluntarily release, waive, and hold harmless NUI Nails & Spa, its owners, managers, employees, independent contractors, technicians, affiliates, successors, and assigns from any and all claims, liabilities, demands, actions, damages, costs, expenses, or losses arising from or related to the services I receive, including but not limited to:

  • Allergic reactions
  • Skin or nail irritation
  • Product sensitivity
  • Cosmetic dissatisfaction
  • Infections
  • Injury or damage resulting from undisclosed allergies or medical conditions
  • Damage caused by failure to follow aftercare instructions
  • Normal wear and tear of nail services
  • Any indirect, incidental, or consequential damages arising from cosmetic services

I understand that this waiver does not release NUI Nails & Spa from liability for conduct that cannot legally be waived, including gross negligence, reckless misconduct, or intentional wrongdoing where prohibited by law.

No Guarantee of Results

I understand that every client responds differently to cosmetic services. Therefore, NUI Nails & Spa makes no guarantee or warranty, express or implied, regarding the longevity, appearance, or outcome of any service provided.

Aftercare Responsibility

I acknowledge that proper aftercare is essential for maintaining the quality and longevity of my service.

I agree that NUI Nails & Spa is not responsible for lifting, chipping, breakage, allergic reactions, discoloration, infections, or other complications resulting from:

  • Failure to follow aftercare instructions
  • Improper home care
  • Accidents
  • Misuse or abuse of the nails
  • Exposure to harsh chemicals
  • Normal daily wear and tear

Refund Policy

I understand that due to the nature of cosmetic services, all services rendered are final and non-refundable.

If I experience a concern related to my service, I agree to notify NUI Nails & Spa within 48 hours of my appointment so that my concern may be evaluated. Any corrective service will be offered solely at the discretion of management.
